Insurance

I currently accept the following insurance plans:

  • HMSA (HMO, PPO, and Federal Employee Program / FEP plans)

  • University Health Alliance (UHA)

  • Hawaii Medical Assurance Association (HMAA)

  • TRICARE (TriWest)

I am not contracted with Medicaid (Quest) and Medicare plans.

Self-pay

If you have another insurance plan, you are welcome to self-pay for visits.

I can provide a superbill for possible out-of-network reimbursement.

My current self-pay rates are:

  • $400 for a new-patient intake (60 minutes)

  • $250–$300 for follow-up visits, depending on length and services provided
